During an extremity exam you routinely include a neurological assessment to check how well the nerves and muscles are functioning. This helps identify injuries to nerves or nerve roots, compressions, or other neurologic problems that could affect strength, sensation, or reflexes in the limb. A typical neuro portion includes motor strength testing, sensation to light touch or pinprick, reflexes, and coordination, and may also assess proprioception and, for the lower extremities, gait. Because of this, saying a neurological exam is never performed is incorrect. In some very focused situations you might do only a quick screen, but it is not accurate to say it is never done.
